The authors report two cases of Histoplasma duboisii pneumonitis in two immunocompetent patients. The underestimated frequency of this disease and the non-specificity of the respiratory radioclinical signs mean that practitioners think little about it. Diagnoses were late because the disease was mistaken for other more frequent diseases such as tuberculosis or trailing bacterial lung diseases. Practitioners should consider this when tests for tuberculosis are not positive and when there is no clinical and radiological improvement despite antibiotic or anti-tuberculosis treatments initiated as a first-line treatment. The exploration of skin lesions, often associated, particularly the lymph nodes, facilitated the diagnosis.
