Histoplasmosis in immunocompetent subjects: report of 2 cases of Histoplasma duboisii infection in Bouaké (Ivory Coast)

Résumé

The authors report two cases of Histoplasma duboisii pneumonia in two immunocompetent patients. The underestimated frequency of this disease and the nonspecificity of its radioclinical respiratory signs mean that clinicians rarely consider it. Diagnoses were often delayed because the condition was mistaken for other, more common illnesses such as tuberculosis or persistent bacterial pneumonia. Clinicians should consider this diagnosis when tests for tuberculosis are negative and in the absence of clinical and radiological improvement despite first-line antibiotic or antituberculosis treatment. Examination of often associated skin lesions, particularly lymph node involvement, facilitates diagnosis.
